Part B premium rates are based on the projected cost to the federal government and Congress, an unpredictable factor. Historically, premiums go up each year. For example, Medicare Part B cost recipients $5.30 in 1970. By 1973, it was up to $6.30, though it was reduced to $5.80 in July and $6.10 in August that year. The premiums continued to increase and reached $31.90 per month in 1989. That rate came from the Medicare Catastrophic Coverage Act of 1988, a change that was intended to expand the program to cover some prescription medications and reduce out-of-pocket costs. It was repealed in 1989.

Treasury loan limits Medicare Part B premium increase

The budget deal reduces the rate of increase through a loan from the Treasury to the Supplementary Medical Insurance (SMI) Trust Fund, but requires the affected individuals to pay back that loan over time. Specifically, seniors not held harmless will pay a base premium of $118.80 a month in 2016 – essentially the amount the Part B premium would have been for all beneficiaries had the hold harmless provision not applied. The difference between $118.80 and $159.30 would be covered by the Treasury loan. To repay the loan, the affected seniors will pay an additional $3 dollars per month, beginning in 2016, until the loan is repaid. With this additional charge, the full base premium for 2016 is $121.80. Medicare beneficiaries who currently pay higher income-related premiums would pay more than $3 per month toward repaying the loan.

MPPR is a per-day policy that applies across disciplines and across settings. For example, if an SLP and a physical therapist both provide treatment to the same patient on the same day, the MPPR applies to all codes billed that day, regardless of discipline. Under MPPR, full payment is made for the therapy service or unit with the highest practice expense value (MPFS reimbursement rates are based on professional work, practice expense, and malpractice components) and payment reductions will apply for any other therapy performed on the same day. For the additional procedures provided on the same day, the practice expense (i.e., support personnel time, supplies, equipment, and indirect costs) of each fee will be reduced by 50% (effective April 1, 2013) for Part B services in all settings.
