The Portland Business Journal reported on the findings in its online edition this week, citing a study by the Institute of Medicine that concluded post-hospital services account for most of the variation in Medicare spending. The study pointed to uneven spending on nursing homes, long-term care hospitals, home health care and inpatient rehabilitation as responsible for 73 percent of the variability in average Medicare spending state-by-state.

Look at the case of two women enrolled in plans across the country from each other. Norma Noriega, 71, of Miami, enrolled in the Leon Cares plan, pays no additional premium beyond the standard Medicare monthly fee of $96.40 a month.  She doesn’t need a Medigap policy. Under traditional Medicare, she would have to pay a $1,100 deductible for her first 60 days in the hospital and 20 percent co-payment for a doctor visit or outpatient care. With her Advantage plan, she pays nothing.

Medicare will pay for dental services that are an integral part of either a covered procedure (e.g., reconstruction of the jaw following accidental injury), or for extractions done in preparation for radiation treatment for neoplastic diseases involving the jaw. Medicare will also may payment for oral examinations, but not treatment, preceding kidney transplantation or heart valve replacement, under certain circumstances. Such examinations would be covered under Part A if performed by a dentist on the hospital’s staff or under Part B if performed by a physician.

Dentists, lawmakers want children’s dental health plan protected

"Removing dental from the SMMC program would harm Medicaid recipients and taxpayers, and would potentially cut services to Medicaid recipients, particularly to vulnerable disabled adults," she wrote in an email to the Herald/Times. "As part of the procurement, the Agency negotiated for adult dental benefits valued at over $100 million over the life of the 5-year contract, all at no cost to taxpayers. To offer this, the plans in SMMC apparently intend to leverage their pediatric dental networks to provide this service to adults cost effectively. If pediatric dental is carved out, SMMC plans will no longer have existing dental networks to leverage and may have to curtail or drop their adult dental coverage."

Houston We Have a Problem

First though, I have to laugh about a lot of conversations I have with dentists about compliance and their need to ensure they have systems in place to protect their dental practices, and what those systems even look like. Some dentists look at me like I’m crazy (right on there). The problem is they give me all kinds of avoidance tactics (B.S.) about how they are good and don’t need me or they have a compliance person. Hey I’m not offended, you’re just not up-to-speed and your ‘compliance’ person doesn’t know crap (I would expect you to tell me I don’t know crap about dentistry, which is true, so we’re even)!

The Australian Dental Association has consistently voiced opposition to the change.
